Yes! We are accepting New Patients!
Dr. Chi is accepting new patients! 
Please note our appointments book approximately 2 weeks out, and our Saturdays book approximately 3-4 weeks out.
First appointments, evening and Saturday hours book farther in advance due to the popularity of appointment times.
We have a <48 hour cancellation fee of $50 that applies to all appointments made per individual.


We are closed on Fridays, Sundays, and all major holidays.

Only $100 for an examination/consultation, x-rays, and a basic cleaning*.
*Basic cleaning only if you qualify based on diagnosis.
*Deep cleanings are NOT included in the New Patient special. A basic cleaning is not provided if a deep cleaning is necessitated.

Phase 5 Update (June 11, 2021)
Per Gov. JB Pritzker's guidelines for Phase 5 in Chicago, all individuals, including those who are fully vaccinated, must continue to wear a mask in healthcare settings.
Therefore, all persons entering and exiting our office should be masked, regardless of vaccination status. Thank you so much for your consideration and cooperation!


A SPECIAL MESSAGE RE: COVID-19

We are open!
Currently we are open on our regular office days (Monday, Tuesday, Wednesday, Thursday, and Saturday) by appointment only during the current phase of our re-opening schedule. Hours will be variable as we are working with our staff schedules during this pandemic. We thank you for your flexibility and understanding as our hours will vary during this time.

We are always closed Fridays and Sundays per usual.

For your future scheduled appointment:
Health and safety are our highest priorities at Chicago Ave Smiles. While we have always gone above and beyond the standard precautions in the office, we are implementing even more measures to maintain a safe and clean workspace by updating our office policies and protocols. 
We appreciate your patience and cooperation in keeping everyone safe.

New office policies for your appointment may include but is not limited to:
- all persons entering the office must have a proper facial covering over nose and mouth
- allowing only patients with an appointment in the office (Minors must be accompanied by one legal parent/guardian).
- contactless temperature reading before being seated in our operatories for treatment
- pre-mouthrinse prior to dental treatment
- maintaining social distancing efforts in the office
- Your dental team at Chicago Ave Smiles will be in additional PPE for protection of the staff and for the protection of patients as well. There is additional equipment present in the treatment rooms and throughout the office as well as added cleaning protocols between patients. These include things such as air purifiers with HEPA filters under proper recommendations by the CDC, a fogger that mists the entire treatment room as an additional measure to disinfect the air and surfaces, new equipment aimed at minimizing aerosols, and even changes in technique for some of our dental procedures. Furthermore, we have made modifications to our scheduling system in an attempt to minimize the number of persons in our office at the same time in the effort of social distancing. 


If you have an appointment in the near future, please reschedule if:
   * you have tested positive for coronavirus within the last 15 days 
   * you have symptoms of illness - particularly coughing, runny nose or sneezing, respiratory illness, fever, and/or loss of taste or smell. Additionally, please contact your medical provider for further information.

    * you have had close contact with someone with a suspected or confirmed case of COVID-19.
    * significantly medically compromised and your scheduled visit is non-essential. If you are immunocompromised and have an urgent dental condition, we recommend that you try to schedule your appointment at the first hour of our business day.
     * you have recently travelled out-of-state that requires a 14-day quarantine. Please refer to the City of Chicago updates.

The above considerations apply for anyone who may accompany you to your visit.  However, in the effort of social distancing and doing our best to keep everyone safe and healthy, our new office policy indicates that only the patient come to the office for appointments (and in the case of a minor, the minor and one legal parent or guardian.)

If you experience any covid-19 symptoms or test positive for coronavirus up to 14 days after your dental appointment, please be sure to inform us within 2 days of your appointment so that we may take appropriate action to keep our patients and staff safe.

​Your patience, help and understanding are needed as we take additional precautions including rescheduling visits as necessary. 
​We appreciate your flexibility during these extraordinary circumstances.
